I agree to indemnify, hold harmless, waive, and relinquish any and all claims I may have against Our Lady of Czestochowa and its officers, agents, employees, representatives, or volunteers arising out of, or in connection with, the SZKOLA POLSKA MICKIEWICZA program in which my child/ward participates while on or about the premises of Our Lady of Czestochowa and adjoining areas, except for claims arising out of the sole negligence or willful and wanton misconduct of Our Lady of Czestochowa and its employees or representatives. Classes will be held in a room with the door open or with a windowed door. All teachers have completed criminal background checks and are complying with state requirements, including viewing required training films and completing a specified class.
